When the botulinum toxin enters the nerve cells it permanently binds to proteins that prevent the release of the neurotransmitter acetylcholine. The nerves fire but acetylcholine doesn't make it across the synapse. The muscles don’t get the signal and don’t contract. The result is a flaccid paralysis that begins at the head and descends. It can eventually cause the patient to die from respiratory failure.
